All inpatient services require precertification with Aspirant. The Inpatient Services Preauthorization Form must be completed and faxed along with the clinical information to the number on the form.
Some outpatient services require precertification (such as MRI, Therapy, etc.). If you are requesting a precertification for an outpatient procedure , the Outpatient Services Precertification Request form must be completed. Please include clinical information and fax to the number on the form.
